1. Who I am
Body Motion Therapy (ABN 43 487 455 469) is a solo practice run by Georges Adam at 5B 195 Somerton Road, Coolaroo VIC 3048. I handle personal information in line with the Privacy Act 1988 (Cth), including the Australian Privacy Principles, and the Health Records Act 2001 (Vic). 2. What I collect
- Contact and booking details: your name, phone number, email address and appointment times.
- Health information: your health history, symptoms, medications, treatment notes and consent records. I collect only what I need to treat you safely.
- Payment and health fund details when you pay or claim a rebate.
- Website information: pages visited, button clicks (such as Book Now), and device and browser type, collected through Google Analytics.
3. How I collect it
I collect information directly from you: through online booking, forms you complete, phone, email and in the clinic. With your consent, I may also collect information from other health practitioners you ask me to contact.
4. Why I collect it
To book and provide treatment, send appointment reminders, keep clinical records, process payments and health fund claims, meet legal obligations, and understand how my website is used.
5. Systems and service providers
I use the following to run the practice. Some of them store or handle your information for me:
- SimplyBook, for online booking. Booking reminder emails are sent automatically.
- JotForm, for forms you complete.
- Google (Gmail, Google Sheets and Google Drive), where appointment records, treatment notes and email are kept.
- Windsor.ai, a reporting tool connected to a Google Sheet.
- HICAPS, for payments and health fund claims at the clinic. If you claim a rebate, the service and your fund membership details are sent to your health fund.
- Website hosting and Google Analytics.
I don’t sell your information. I share it with others only with your consent, where the law requires or allows it, or to prevent a serious threat to someone’s health or safety.
6. Information stored outside Australia
Some of these providers may store or process information outside Australia. I rely on each provider’s own privacy and security terms and take reasonable steps to make sure your information is protected.
7. Marketing
I don’t use your health information, treatment details, images or reviews in my marketing, and I won’t publish anything that identifies you without your written consent. Appointment reminders are not marketing.
8. Website analytics
I use Google Analytics to understand how the website is used. You can block cookies in your browser settings or use Google’s opt-out tools. The booking system and forms have their own privacy settings.
9. Security and how long I keep records
I take reasonable steps to protect personal information from misuse, loss and unauthorised access. Under Victorian law I keep health records for at least 7 years after your last visit, or until a child turns 25, and then securely destroy or de-identify them.
10. Access and correction
You can ask me for a copy of your information or ask me to correct it. I will respond within 30 days.
11. Complaints
Please contact me first. If the matter isn’t resolved, you can contact the Office of the Australian Information Commissioner (oaic.gov.au, 1300 363 992) or the Health Complaints Commissioner Victoria (hcc.vic.gov.au, 1300 582 113).
12. Changes
I will post any updates to this policy on this page.
